Apply for rehabilitation within the earnings-related pension scheme on the forms below. You need to enclose a medical statement B2 issued by an occupational health doctor or other doctor responsible for your care concerning your state of health and remaining functional capacity and capacity to work.

Decision concerning the application

Based on the documents, an assessment is made as to whether you are entitled to rehabilitation within the earnings-related pension scheme.

If you fulfil the criteria that apply to vocational rehabilitation, you receive a preliminary decision concerning your entitlement to vocational rehabilitation as well as the estimated amount of your rehabilitation allowance. The decision specifies the date until which it is valid.

If you did not send your rehabilitation plan with your application, you must submit a plan while the preliminary decision is still valid otherwise, the preliminary decision concerning vocational rehabilitation will expire. A separate decision is issued when the plan for vocational rehabilitation has been drawn up. The decisions are issued at the same time if a jointly approved plan already exists when you apply for rehabilitation.

You are entitled to appeal for changes if you are dissatisfied with the decisions.
